Our program works directly with the caregiver, and we require the caregiver’s accurate contact details to process and communicate about the application.

To get started:

1. Complete this form and upload all requested documents.
2. The caregiver’s full and accurate contact information is required for this program. Please carefully verify the caregiver’s email address and phone number before submitting, as all communication will be sent directly to the caregiver.

After submission:

• You will receive a confirmation email indicating that your application has been successfully submitted. If you do not receive this email within 24 hours, please resubmit your application.
• Within 5 business days, you will receive a follow-up email from a “@qolfoundation.org” email address. If you do not see this message, please check your spam or junk folder.

To ensure a safe and supportive environment for everyone, we maintain a zero-tolerance policy for rude, abusive, or threatening behavior toward staff. Violation of this policy may result in immediate termination of assistance.
